ProMedica Toledo Hospital is one of the largest acute-care facilities in the region. Provides advanced pharmacy services, including drug information, pharmacokinetics, drug therapy monitoring, patient education, medication reconciliation, transition of care evaluations, and ensures compliance with controlled substance regulations. Responsible for directly supervising pharmacy support personnel and actively engaging and collaborating with all hospital personnel. Education: PharmD degree or BSPharm with relevant experience post- 2006.
Skills: Experience in hospital pharmacy services, including unit dose, IV admixtures, medication reconciliation, and addressing provider questions.
Years of Experience: Minimum 1 year (full-time equivalent of 2,040 hours) in inpatient pharmacy practice.
License: License: Must be a registered pharmacist, licensed in good standing in Michigan and Ohio. Single facility based pharmacists must retain state licensure per site location.
Certification: BLS (Basic Life Support)
